Select the desired form from one of the three links to its right.

• The “Word doc with fill-ins” is a Microsoft Word format, designed to allow you to fill in the blanks on your computer.
• The “Word document” is the form in Microsoft Word format.
• The “PDF document” is the form in Adobe Acrobat format. You need the Acrobat reader to display or print it.
• The “Excel document” is the form in Microsoft Excel format.

The preferred approach is to download the Word doc with fill-ins, fill in the blanks, save the resulting document, and email it to stpatschedules@aol.com. You can print any of those forms and fill it in, then deliver it to the Rectory.
